1. Where can I get information confirming the fact of my place of residence registration?

At the Portal of Electronic Services of the city of Kharkov or by applying to the authorized bodies of certificates issuance:

  • 11 Kharkiv Administrative Services Provision Centers;
  • 66 divisions of Communal Enterprise "Zhilkomservis".

2. What is needed to get information about yourself from Kharkiv Territorial Community Register at Kharkiv Administrative Services Provision Centers or its territorial subdivisions?

To receive personal information from Kharkov Territorial Community Register, the person must apply to Kharkiv Administrative Services Provision Center or its territorial subdivisions with a written application (filled in in the ASPC) in the form established by the registrating institution. The written application shall be attached with:

  • a copy of the document authenticated by the applicant and certifying the identity of the his person, with the submission of the original and relevant document to the administrator of the Center (for persons who, in accordance with the legislation requirements must receive it);
  • written consent for the processing of personal data.

2a What do I need to get information about myself from Kharkiv Territorial Community Register at the divisions of the Communal Enterprise "Zhilkomservis"?

To receive information about yourself from the Kharkiv Territorial Community Register, the person must contact any office of the Communal Enterprise "Zhilkomservis" with a written request (filled in in the district) in the form established by the registrating authority. The written application shall be attached with:

  • a copy of the document authenticated by the applicant and certifying the identity of the his person, with the submission of the original and relevant document to the administrator of the Center (for persons who, in accordance with the requirements of the legislation, shall receive it);
  • written consent for the processing of personal data.

3. How can I receive my personal information from Kharkiv Territorial Community Register via Kharkiv Electronic Services Portal?

To receive personal information from Kharkiv Territorial Community Register via Kharkov Electronic Services Portal, you should authorize by means of remote identification (electronic digital signature, BankId), click on the "Order" button and fill in the required fields. After entering all the data, you need to press the "order" button.

4. In what form will I receive information about myself from Kharkov Territorial Community Register?

Information about yourself from Kharkov Territorial Community Register is provided:

  • in the form of a certificate of registration of the place of residence (Appendix 13 to the Rules for registration of residence approved by the Cabinet of Ministers of Ukraine from 02.03.2016 № 207) in paper form in case of a personal applying through the Administrative Services Provision Center of the city of Kharkov or its territorial subdivisions, "Zhilkomservice" communal enterprise divisions.
  • in the form of a place of residence registration certificate (Appendix 13 to the Rules for residence registration approved by the Cabinet of Ministers of Ukraine from 02.03.2016 № 207) in electronic form in case of identification of the person by means of remote identification (electronic digital signature, etc.) through the Electronic Portal Services of the city of Kharkov.

5. What should I do if there is no information about me in Kharkov Territorial Community Register when applying for information about myself from Kharkov Territorial Community Register?

A person shall apply to Kharkiv Administrative Services Provision Center or its territorial subdivisions with a written request (to be filled in at ASPC) for entering actual information in Kharkov Territorial Community Register. With written application the following documents shall be attached:

  • a document in which information about the place of residence / location of a person (for persons who in accordance with the requirements of the law should receive it) is entered, and a certified copy thereof. Persons under 16 years of age, before filing documents, not being documented in the document in which information about the place of residence / location of a person is added, submit a birth certificate;
  • a document confirming information of registration / withdrawal of the registration of the residence place / location of the person in the city of Kharkov (for a juvenile/ minor - in case of existence).

In case of submitting an application by a representative of the person additionally shall be submitted:

  • certified by the applicant copy of the document proving his identity, with the presentation of the original of the relevant document to the administrator of the Center;
  • a document confirming the authority of the person as a representative, except where the legal representatives are the parents (adoptive parents).

6. What should I do in case of erroneous information about me in Kharkov Territorial Community Register when applying for information about myself from Kharkov Territorial Community Register?

The person must apply to Kharkiv Administrative Services Provision Center or its territorial subdivisions with a written application (filled in at the ASPC) to amend the information contained in Kharkov Territorial Community Register.

The written application shall be accompanied by:

  • a document to which the information about the place of residence / location of the person is entered (for persons who according to the requirements of the legislation should receive it) and its certified copy. Persons under the age of 16 years who before the submission of documents are not documented with a document to which the information about the place of residence / location of the person is entered, shall submit a birth certificate;
  • a document confirming the registration / withdrowal of registration of the place of residence / location of the person in Kharkiv city (for a juvenile / minor - in case of existence);
  • a document confirming the accuracy of the information provided by the applicant, in case of information not being confirmed by the document to which the information about the place of residence / location of the person is entered (for example: marriage certificate, certificate of change of name / surname, etc.);
  • a document confirming the change in the personal data of the person in case of information not being confirmed by the document to which the information about the place of residence / location of the person is entered.

In the case of an application submission by a representative of the person additionally attached:

  • a copy of a document certifying the identity of the applicant certified by the applicant, with the submission of the original of the relevant document to the administrator of the Center;
  • a document confirming the authority of a person as a representative, except for cases when the legal representatives are the parents (adopters).

The deadline for reviewing documents is 1 day, if necessary, the deadline for consideration of the relevant documents may be extended to 20 working days, about what the person is notified additionally.

7. What is necessary for obtaining information about a person from Kharkov Territorial Community Register by a representative / legal representative?

For receiving information from Kharkiv Territorial Community Register, the representative / legal representative of the person whose interests are represented, you must apply to the authorized certificates issuing agencies: Kharkiv Administrative Services Provision Center or its territorial subdivisions, "Zhilkomservis" communal enterprise division with a written application ( is filled in at the place) in the form established by the registration authority. The written application shall be attached with:

  • a copy of a document certifying the applicant's identity certified by the applicant, with the submission of the original and relevant document to the Center's administrator / employee of the district of the communal enterprise "Zhilkomservis";
  • power of attorney (certified in accordance with the procedure established by the law) according to which the person whose information is contained in the Register gives the authority to a representative to receive (his/her) personal data / information but for cases when the legal representatives are the parents (adopters) of the person that is confirmed with the relevant document);
  • written consent for the processing of personal data.

8. What are the reasons for the refusal to provide information from Kharkiv Territorial Community Register?

The provision of information from the Register may be refused if:

  • written application is not filed according to the established form;
  • the applicant did not submit the necessary documents or information;
  • the submitted documents contain inaccurate information or the submitted documents are invalid;
  • a person who has not reached the age of 14 has applied;
  • documents are not filed in the order established by this Regulation.

Obtaining an information certificate from Kharkiv Territorial Community Register about persons registered at the address of the dwelling, by the owner / one of the co-owners / tenant of the dwelling, or by the person whose place of residence is registered at the address of the corresponding dwelling, who has reached the age of 14 years.

1. Who can receive the information reference from Kahrkiv Territorial Community Register about persons registered at the address of the dwelling?

The owner, one of the co-owners, the tenant of the dwelling, the person whose place of residence is registered at the address of the corresponding dwelling and has reached the age of 14 years.

2. Where the owner of the appropriate dwelling can obtain information from Kharkiv Territorial Community Register on persons whose place of residence / location is registered at the appropriate address?

In any of the 11 Kharkiv Administrative Services Provision Centers or in any of the 66 divisions of the "Zhilkomservice" Communal Enterprise.

3. Where the co-owner of the corresponding dwelling can receive information from Kharkiv Territorial Community Register concerning persons whose place of residence / location is registered at the appropriate address?

In any of the 11 Kharkiv Administrative Services Provision Centers or in any of the 66 divisions of the "Zhilkomservice" Communal Enterprise.

4. Where the tenant of the corresponding dwelling can receive information from Kharkiv Territorial Community Register about the persons whose place of residence / location is registered at the relevant address?

5. Where the person, whose place of residence is registered at the address of the relevant dwelling, can obtain information from Kharkiv Territorial Community Register about persons whose place of residence / location is registered at the appropriate address?

In any of the 11 Kharkiv Administrative Services Provision Centers or in any of the 66 divisions of the "Zhilkomservice" Communal Enterprise.

6. Can a legal representative / representative under the power of attorney receive an information note from Kharkiv Territorial Community Register about the persons whose place of residence / location is registered at the address?

Yes. For a legal representative, you must additionally file a document confirming the authority of the person as a legal representative. For a representative by power of attorney, additionally a power of attorney must be submitted in accordance with the procedure established by law.

7. How to obtain an information certificate from Kharkiv Territorial Community Register about the persons whose place of residence / location is registered at the address as the owner of the relevant dwelling?

The owner of the appropriate dwelling must contact any Kharkiv Administrative Services Provision Center or in any division of the "Zhilkomservice" Communal Enterprise with a written application in the form established by the registration authority. The written application shall be attached with:

  • a copy of the document certifying the identity of the applicant certified by the applicant, with the submission of the original of the relevant document to the authorized body for certificates issue from the Register;
  • certified by the authorized body for certificates issue from the Register, a copy of the document confirming the applicant's ownership of the relevant home, with the submission of the original of the mentioned document to the authorized body for certificates issue from the Register.

8. How the co-owner of the dwelling can get an information note from Kharkiv Territorial Community Register about the persons whose place of residence / location is registered at the address?

The co-owner of the appropriate dwelling must contact any Kharkiv Administrative Services Provision Center or any division of the Zhilkomservice" Communal Enterprise with a written application in the form established by the registration authority. The following documents shall be attached to the written application:

  • a copy of the document certifying the identity of the applicant certified by the applicant, with the submission of the original and relevant document to the authorized body for the certificates issue from the Register;
  • certified by the authorized body for certificates issue from the Register, a copy of the document confirming the applicant's ownership of the relevant dwelling, with the submission of the original of the mentioned document to the authorized body for certificates issue from the Register.

9. How can a tenant of a corresponding dwelling receive an information certificate from Kharkiv Territorial Community Register about persons whose place of residence / location is registered at this address?

The tenant of the corresponding dwelling must contact any Kharkiv Administrative Services Provision Center or any division of the Zhilkomservice" Communal Enterprise with a written application in the form established by the registration authority. To the written request should be attached:

  • A copy of a document certifying his identity, certified by the applicant, with the original of the relevant document being presented to the authorized body for certificates issuance from the Register;
  • certified by the authorized body for certificates issuance from the Register, a copy of the document confirming the status of the tenant of dwelling: an order, a lease contract (sublease, rent), the decision of the authorized body on the recognition of the person as a tenant, with the original of the specified document being presented to the authorized body for certificates issuing from the Register.

10. How can a person whose place of residence have been registered at the address of the relevant dwelling and who reached the age of 14 obtain an information certificate from Kharkiv Territorial Communities Register about persons whose place of residence / location is registered at the address?

A person whose place of residence is registered at the address of the mentioned dwelling and reached the age of 14 should contact Kharkiv Administrative Services Provision Center or any division of the Zhilkomservice" Communal Enterprise with a written application in the form established by the registration authority. To the written request are attached:

  • - a copy of the document in which information about the place of residence is entered, with the original of the document being presented to the authorized body for certificates issuance from the Register.

11. How a legal representative of the owner / co-owner / tenant / person whose place of residence is registered at the address of the relevant dwelling and who reached the age of 14, can obtain an information certificate from Kharkiv Territorial Communitiy Register about persons whose place of residence / stay is registered at?

Legal representatives of owners / co-owners / employers / persons whose place of residence is registered at the address of the corresponding dwelling and who reached the age of 14 must contact Kharkiv Administrative Services Provision Center or any division of the Zhilkomservice Communal Enterprise with a written application in the form established by the registration authority. To the written request are attached:

  • certified by the applicant copy of the document proving his identity, with the presentation of the original of the specified document to the authorized body for certificates issuance from the Register;
  • certified by the authorized body for issuing certificates from the Register, сopy of the document confirming the applicant's ownership of corresponding dwelling, with the original of the mentioned document being presented to the authorized body for certificates issuance from the Register (for the owner (co-owners) of dwelling)
  • certified by the authorized body for certificates issuance from the Register, copy of the document confirming the status of the tenant of dwelling: the warrant, the contract of hiring (sublease, lease), the decision of the authorized body on the recognition of the person to be a tenant, with the presentation of the original of the specified document to the authorized body for issuing certificates from the Register;
  • a copy of the document certified by the applicant in which information about the place of residence is entered, with the presentation of the original of the specified document to the authorized body for certificates issuance from the Register (for the person whose residence is registered at the address of the corresponding dwelling);
  • certified by the applicant (legal representative) copy of the document confirming the authority of the person as a legal representative, with the presentation to the authorized body for certificates issuance from the Register of the original of this document.

12. How an owner's representative by trust / co-owner / tenant / person whose place of residence is registered at the address of the relevant dwelling and who reached the age of 14, can obtain an information certificate from the Kharkiv Territorial Community Register about the persons whose place of residence / location is registered at?

Representative of the owner by trust / co-owner / employer / person whose place of residence is registered at the address of the corresponding dwelling and has reached the age of 14 must contact any Kharkiv Administrative Services Provision Center or any division of the "Zhilkomservice" Communal Enterprise with a written application in the form established by the registration authority. To the written request are attached:

  • a copy of the document certifying his identity, certified by the applicant, with the submission of the original of the mentioned document to the authorized body for certificates issuance from the Register;
  • a copy of the document confirming the applicant's right to the corresponding accommodation, certified by the authorized body for certificates issuance from the Register, with the submission of the original of the mentioned document to the authorized body for certificates issuance from the Register (for the owner (co-owners) of the dwelling)
  • a copy of the document confirming the applicant's status of the tenant, certified by the authorized body for certificates issuance from the Register: a warrant, a contract of tenancy (sublease, lease), a decision of the authorized body on the recognition of the person as tenant, with the presentation of the original document of the mentioned document to the authorized body for certificated issuance from the Registry;
  • a copy of the document certified by the applicant, in which the information about the place of residence is entered, with the submission of the original document to the authorized body for certificates issuance from the Register (for the person whose place of residence is registered at the address of the mentioned dwelling);
  • power of attorney certified in accordance with the procedure established by law (in case of submission of documents by the representative).

13. What to do in case of absence of information about me in the information reference from Kharkiv Territorial Community Register on persons whose place of residence / location is registered at the address of the dwelling?

The person whose information is absent in Kharkov Territorial Community Register should contact the Kharkiv Administrative Services Provision Center or any division of the Zhilkomservice Communal Enterprise with a written application in the form established by the registration authority.To the written application are attached:

  • a document in which information about the place of residence / location of a person (for persons who in accordance with the requirements of the law should receive it) is entered, and a certified copy thereof. Persons under 16 years of age, who were not before the submission of documents documented by a document in which information on the place of residence / location entered, submit a birth certificate;
  • a document confirming information on registration / withdrawal of registration of the place of residence / location of the person in the city of Kharkov (for a juvenile / minor person, if there are any).

In the case of filing an application by a representative of the person shall additionally submit:

  • certified copy of the document proving his identity, with the presentation of the original of the relevant document;
  • a document confirming the authority of the person as a representative, except where the legal representatives are the parents (adoptive parents).

14. What should I do if I find false information about me in the information reference from Kharkov Territorial Communities Register about the persons whose place of residence / location is registered at the address of dwelling?

The person whose false data is contained in the Register should contact Kharkiv Administrative Services Provision Center or its territorial subdivisions, with a written request (to be filled in place) for making changes to the information contained in Kharkov Territorial Communities Register. To the written request are attached:

  • a document in which information about the place of residence / location of the person is entered (for those persons who according to the requirements of the legislation must receive it) and its certified copy. Persons under the age of 16 years who before the date of submission of documents are not documented by a document in which the information about the place of residence / stay is submitted, submit a birth certificate;
  • a document confirming the registration / withdrawal from registration of the place of residence / location of the corresponding person in the city of Kharkov (for a juvenile/ minor person - in case of existence);
  • a document confirming the credibility of the information declared by the applicant, in case of not being confirmed by the document, where the information about the place of residence / location of the person is entered;
  • a document confirming the change of the personal data of the person in case of not being confirmed by the document, in which the information about the place of residence / location of the person is entered.

In the case of filing an application by a representative of the person shall additionally submit:

  • a certified copy of the document certifying his identity, with the presentation of the original of the corresponding document;
  • a document confirming the authority of the person as a representative, except when the legal representatives are the parents (adoptive parents).

15. What to do if, in the information note from Kharkov Territorial Community Register, the persons whose place of residence / location is registered at the address of the dwelling indicated a person whose place of residence is not registered at the appropriate address?

In case of revealing of inaccurate information obtained from Kharkov Territorial Community Register, a person must fill in a written application for adjusting the data in Kharkov Territorial Community Register at Kharkiv Administrative Services Provision Center or its territorial divisions or by calling the hotline number 0 800 304 304 specified in relevant reference or submit an e-mail application to the Kharkiv Electronic Services Portal at www.e-kharkiv.org, for adjusting the data in Kharkiv Territorial Community Register.

16. What to do if in the information reference from Kharkov Territorial Community Register about the persons whose place of residence / location is registered at the address there is a duplicate of the same person at the mentioned address?

In case of revealing of inaccurate information obtained from Kharkov Territorial Community Register, a person must fill in a written application for adjusting the data in Kharkiv Territorial Community Register at Kharkiv Administrative Services Provision Center or its territorial divisions or by calling the hotline number 0 800 304 304 specified in relevant reference or submit an e-mail application to the Kharkiv Electronic Services Portal at www.e-kharkiv.org, for adjusting the data in Kharkov Territorial Community Register.

17. What to do if in the information certificate from Kharkov Territorial Community Register, the owner / co-owner / tenant of the corresponding dwelling received information that the indicated person removed from the registration of the place of residence at the mentioned address?

In case of revealing of inaccurate information obtained from Kharkov Territorial Community Register, a person must submit a written application for adjusting the data in Kharkiv Territorial Community Register at Kharkiv Administrative Services Provision Center or its territorial divisions or by calling the hotline number 0 800 304 304 specified in relevant reference, or submit an e-mail application to the Kharkiv Electronic Services Portal at www.e-kharkiv.org, for adjusting the data in Kharkiv Territorial Community Register.

18. What is the deadline for consideration of the application (oral, written, electronic) for adjusting the information contained in Kharkiv Territorial Community Register?

The period of consideration of the application (written, electronic) for the correction of the information contained in Kharkiv Territorial Community Register, is 1 day, if necessary, the period for consideration of the relevant appeal can be extended to 20 working days, that is notified to the person additionally.

19. Is it possible to obtain an information certificate from Kharkiv Territorial Community Register about persons whose place of residence / location is registered at the address of the dwelling on a certain date?

Yes, in order to receive an information certificate from Kharkiv Territorial Community Register about persons whose place of residence / location is registered at the address of the dwelling on a certain date, the owner, one of the co-owners, the tenant or a person whose residence is registered at the address of the corresponding dwelling; who has reached the age of 14, a legal representative or representative by proxy, must apply to any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise with a written application in the form established by the registration authority. To the written request attached:

  • certified by the applicant a copy of the document proving his identity, with the presentation of the original of the mnetioned document to the authorized body for certificates issuance from the Register;
  • certified by the authorized body for certificates issuance from the Register, a copy of the document confirming the applicant's ownership of corresponding dwelling, with the original of the specified document being presented to the authorized body for certificates issuance from the Register (for the owner (co-owners) of dwelling)
  • certified by the authorized body for certificates issuing from the Register, copy of the document confirming the status of the tenant: a warrant, a lease contract (sublease, lease), the decision of the authorized body to recognize the person as the employer, presenting the original of the specified document to the authorized body for certificates issuing from the Register;
  • a copy of the document certified by the applicant in which information about the place of residence is entered, with the presentation of the original of the specified document, to the authorized body for certificates issuing from the Register (for the person whose place of residence is registered at the address of the corresponding dwelling);
  • certified by the applicant (legal representative) copy of the document confirming the authority of the person as a legal representative, with the presentation to the authorized body for certificates issuance from the Register of the original of this document;
  • the power of attorney certified in the order established by the legislation (in case of presentation of documents by the representative).

20. Why the certificate does not contain the the data of registered persons at the corresponding address?

Kharkiv Territorial Community Register was formed on the basis of information received from the Department of Registration of the Kharkov City Council from the authority of the State Register of Voters in accordance with Section II, paragraph 2 of Law No. 888-VIII. In this regard, 04/04/2016 registered citizens of Ukraine who have reached the electoral age are in Kharkiv Territorial Community Register. Therefore, information concerning the registered place of residence of citizens under the age of 18 was not contained in Kharkiv Territorial Community Register .

Please note that the details of the passport data and the date of registration of residence are not available in the State Register of Voters, therefore after 04.04.2016 uploaded data to Kharkiv Territorial Community Register, this information is not available.

Obtaining a certificate of the persons registered in the dwelling (family status certificate) by the owner / one of the co-owners / tenant of the accommodation, or by the person whose place of residence is registered at the address of the corresponding accommodation, reached the age of 14 years.

1. Who can receive a certificate of the persons registered in a residential dwelling (a certificate about the composition of the family) of the relevant address?

The owner, one of the co-owners, the tenant, the person whose place of residence is registered at the address of the relevant dwelling and has reached the age of 14 years.

2. Where the owner of the relevant dwelling can receive a certificate about the persons registered in the apartment (family status certificate) of the relevant address?

In any of the 11 Kharkiv Administrative Services Provision Centers or in any of the 66 divisions of "Zhilkomservice" Communal Enterprise.

3. Where the co-owner of the relevant dwelling can receive a certificate about the persons registered in the dwelling (certificate of the family composition) at the relevant address?

In any of the 11 Kharkiv Administrative Services Provision Center or in any of the 66 divisions of "Zhilkomservice" Communal Enterprise.

4. Where is the tenant of the relevant dwelling can receive a certificate of the persons registered in the dwelling (a certificate on the composition of the family) at the relevant address?

In any of the 11 Kharkiv Administrative Services Provision Center or in any of the 66 divisions of "Zhilkomservice" Communal Enterprise.

5. Where the person, whose place of residence registered at the address of the relevant dwelling can receive a certificate of the registered persons in the dwelling (a certificate of family composition) of the relevant address?

In any of the 11 Kharkiv Administrative Services Provision Center or in any of the 66 divisions of "Zhilkomservice" Communal Enterprise.

6. Can a legal representative / representative by proxy get a certificate of a person registered in the dwelling (a certificate of family composition) of the relevant address?

Yes. In order to obtain a certificate, the legal representative must additionally submit a document confirming the authority of the person as a legal representative. Legal representative must additionally submit a power of attorney, certified in accordance with the procedure established by law.

7. How do the owner of the relevant dwelling get a certificate of registered in the dwelling (family composition certificate) of the appropriate address?

The owner of the appropriate dwelling must contact any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise, with a written request in the form established by the registration authority. To the written request attached:

  • a copy of the document certifying the identity of the person certified by the applicant, with the submission of the original of the relevant document to the authorized body for certificates issue from the Register;
  • a copy of the document confirming the applicant's right of the relevant dwelling certified by the authorized body for certificates issue from the Register, with the submission of the original of the specified document to the authorized body for certificates issue from the Register.

8. How a co-owner of the corresponding dwelling can get a certificate about the persons registered in the dwelling (certificate about the composition of the family) of the relevant address?

A co-owner of the corresponding dwelling must contact any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise, with a written request in the form established by the registration authority. To the written request attached:

  • a copy of the document certifying his identity, certified by the applicant, with the submission of the original of the corresponding document to the authorized body for certificates issuing from the Register;
  • certified by the authorized body for certificates issuance from the Register, a copy of the document confirming the applicant's ownership of the relevant dwelling, with the original of the specified document being presented to the authorized body for certificates issuance from the Register.

9. How can a tenant of a corresponding dwelling receive a certificate of a person registered in the dwelling (a certificate of family composition) of the relevant address?

The tenant of the corresponding dwelling must contact any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise, with a written request in the form established by the registration authority. To the written request attached:

  • certified copy of the document proving his identity, with the presentation of the original of the relevant document to the authorized body for certificates issuance from the Register;
  • certified by the authorized body for certificates issuance from the Register, a copy of the document confirming the status of the tenant of dwelling: the warrant, the contract of hiring (sublease, lease), the decision of the authorized body on the recognition of the person be a tenant, with the presentation of the original of the specified document to the authorized body for certificates issuance from the Register.

10. How can a person, whose place of residence is registered at the address of the relevant dwelling and who reached the age of 14, get a certificate of the persons registered in the dwelling (a certificate of the family composition) of the relevant address?

A person whose place of residence is registered at the address of the relevant dwelling and who has reached the age of 14 must contact any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise, with a written request in the form established by the registration authority. To the written request attached:

  • a copy of the document in which information about the place of residence is entered, with the original of the document being presented to the authorized body for certificates issuance from the Register

11. How can a legal representative of the owner / co-owner / tenant / person whose place of residence is registered at the address of the relevant dwelling and reached the age of 14 years obtain a certificate of registered persons in the dwelling (certificate of family composition) of the relevant address?

The legal representative of the owner / co-owner / tenant / people whose place of residence is registered at the address of the relevant dwelling and reached the age of 14 years of the corresponding dwelling must contact any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise, with a written request in the form established by the registration authority. To the written request attached:

  • a certified copy of the document certifying the identity of the applicant, with the submission of the original of the specified document to the authorized body for certificates issue from the Register;
  • a certified copy of the document confirming the applicant's ownership of the relevant dwelling, certified by the authorized body for certificates issue from the Register, with the submission of the original of the specified document to the authorized body for certificates issue from the Register (for the owner (co-owners) of the dwelling);
  • certified by the authorized body for certificates issuance from the Register, a copy of the document confirming the status of the tenant of dwelling: the warrant, the contract of hiring (sublease, lease), the decision of the authorized body on the recognition of the person tp be the tenant, with the presentation of the original of this document to the authorized body for certificates issuing from the Register;
  • a copy of the document certified by the applicant in which information about the place of residence is entered, with the presentation of the original of the specified document to the authorized body for certificates issuance from the Register (for the person whose residence is registered at the address of the corresponding dwelling);
  • certified by the applicant (legal representative) a copy of the document confirming the authority of the person as a legal representative, with the presentation to the authorized body for certificates issuance from the Register of the original of this document.

12. How can a representative by proxy of the owner / co-owner / tenant / person whose place of residence is registered at the address of the relevant dwelling and who reached the age of 14, obtain a certificate of registered persons in the residential dwelling (certificate of family composition) of the relevant address?

The Representative of the owner, under the power of attorney / co-owner / tenant / person whose place of residence is registered at the address of the relevant dwelling and has reached the age of 14 years of the relevant dwelling, should contact any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise, with a written request in the form established by the registration authority. To the written request attached:

  • a certified copy of the document certifying the identity of the applicant, with the submission of the original of the specified document to the authorized body for certificates issue from the Register;
  • a certified copy of the document confirming the applicant's ownership of the relevant dwelling, certified by the authorized body for certificates issue from the Register, with the submission of the original of the specified document to the authorized body for certificates issue from the Register (for the owner (co-owners) of the dwelling);
  • certified by the authorized body for certificates issuance from the Register, a copy of the document confirming the status of the tenant of dwelling: the warrant, the contract of hiring (sublease, lease), the decision of the authorized body on the recognition of the person by the tenant, with the presentation of the original of this document to the authorized body for certificates issuing from the Register;
  • a copy of the document certified by the applicant in which information about the place of residence is entered, with the presentation of the original of the specified document to the authorized body for certificates issuance from the Register (for the person whose residence is registered at the address of the corresponding dwelling);
  • the power of attorney certified in the order established by the legislation (in case of presentation of documents by the representative).

13. What should I do if there is no information about me in the certificate concerning persons registered in the dwelling (the certificate of the family composition) at the relevant address?

The person whose information is absent in Kharkov Territorial Community Register should contact the Kharkiv Administrative Services Provision Center or its territorial subdivisions with a written application (to be filled in at the place) for entrering into Kharkov Territorial Community Register actual information about himself. To the written application attached:

  • a document in which information about a person's place of residence / location is entered (for persons who, in accordance with the requirements of the law, should receive it), and a certified copy. Persons under the age of 16 who, before submitting documents, are not documented by a document in which information on the person's place of residence / location is entered, issue a birth certificate;
  • a document confirming information on registration / withdrawal from the registration of the residence / stay of a person in the city of Kharkov (for a juvenile / minor, in case of existance).

In the case of applying by the representative of the person, he/she shall additionally submit:

  • certified copy of the document proving his identity, with the presentation of the original of the relevant document;
  • a document confirming the authority of the person as a representative, except where the legal representatives are the parents (adoptive parents).

14. What should I do if there are erroneous personal data in the certificate of a person registered in a dwelling (a certificate of the family composition) of the relevant address?

The person whose false data is kept in the Register should contact the Kharkiv Administrative Services Center or its territorial subdivisions, with a written request (to be filled in at the place) for making changes to the information contained in the Kharkov Territorial Communities Register. To the written application attached:

  • a document in which information about the place of residence / location of a person (for persons who in accordance with the requirements of the law should receive it) is entered, and a certified copy thereof. Persons under 16 years of age, who before the documents submission, are not documented in the document in which information concerning the place of residence / location of a person is entered, submit birth certificate;
  • a document confirming information of registration / withdrawal of the registration of the residence place / location of the person in the city of Kharkov (for a juvenile/ minor - in case of existence);
  • a document confirming the authenticity of the information declared by the applicant, if it is confirmed with a document in which information concerning the place of residence / location of the person is entered;
  • a document confirming the change of the personal data of the person, in case of confirmation with the document, in which information about the place of residence / location of the person is entered.

In case of submitting an application by a representative of the person additionally shall be submitted:

  • certified copy of the document proving his identity, with the presentation of the original of the relevant document;
  • a document confirming the authority of the person as a representative, except where the legal representatives are the parents (adoptive parents).

15. What if in the certificate of persons registered in a dwelling (a certificate of family composition) at the relevant address, the person is not registered at the relevant address?

In case of revealing of inaccurate information in the certificate of persons registered in a dwelling (a certificate of family composition), a person must fill in a written application for adjusting the data in Kharkov Territorial Community Register at Kharkiv Administrative Services Provision Center or its territorial divisions or by calling the hotline number 0 800 304 304 specified in relevant reference or submit an e-mail application to Kharkiv Electronic Services Portal at www.e-kharkiv.org, for adjusting the data in Kharkiv Territorial Community Register.

16. What should I do if there is a double of same person at the specified address in the certificate of registered persons in the dwelling (certificate of family composition)?

In case of revealing of inaccurate information in the certificate certificate of persons registered in a dwelling (a certificate of family composition), a person must fill in a written application for adjusting the data in Kharkov Territorial Community Register at Kharkiv Administrative Services Provision Center or its territorial divisions or by calling the hotline number 0 800 304 304 specified in relevant reference or submit an e-mail application to Kharkiv Electronic Services Portal at www.e-kharkiv.org, for adjusting the data in Kharkiv Territorial Community Register.

17. What to do if in the information certificate of persons registered in the dwelling (a certificate of family composition) there is indicated a person removed from the registration (before 04.04.2016) of the place of residence at the specified address?

In case of revealing of inaccurate information in the certificate of persons registered in a dwelling (a certificate of family composition), a person must fill in a written application for adjusting the data in Kharkov Territorial Community Register at Kharkiv Administrative Services Provision Center or its territorial divisions or by calling the hotline number 0 800 304 304 specified in relevant reference or submit an e-mail application to Kharkiv Electronic Services Portal at www.e-kharkiv.org, for adjusting the data in Kharkiv Territorial Community Register.

18. What is the deadline for consideration of the application (oral, written, electronic) for adjusting the information contained in certificate of persons registered in a dwelling (a certificate of family composition) at the specified address?

The period of consideration of the application (written, electronic) for the correction of the information concerning persons registered in a dwelling (a certificate of family composition) at the specified address, is 1 day, if necessary, the period for consideration of the relevant appeal can be extended to 20 working days, that is notified to the person additionally.

19. Is it possible to obtain an information certificate about persons registered in a dwelling (a certificate of family composition) at the specified address on a certain date?

Yes, in order to receive an information certificate about persons registered in a dwelling (a certificate of family composition) at the specified address on a certain date, the owner, one of the co-owners, the tenant or a person whose residence is registered at the address of the corresponding dwelling; who has reached the age of 14, a legal representative or representative by proxy, must apply to any Kharkiv Administrative Services Provision Center or any division of "Zhilkomservice" Communal Enterprise with a written application in the form established by the registration authority. To the written request attached:

  • certified by the applicant a copy of the document proving his identity, with the presentation of the original of the mnetioned document to the authorized body for certificates issuance from the Register;
  • a certified by the authorized body for certificates issuance from the Register, copy of the document confirming the applicant's ownership of corresponding dwelling, with the original of the specified document being presented to the authorized body for certificates issuance from the Register (for the owner (co-owners) of dwelling)
  • certified by the authorized body for certificates issuing from the Register, a copy of the document confirming the status of the tenant: a warrant, a lease contract (sublease, lease), the decision of the authorized body to recognize the person as the employer, presenting the original of the specified document to the authorized body for certificates issuing from the Register;
  • a copy of the document certified by the applicant in which information about the place of residence is entered, with the presentation of the original of the specified document, to the authorized body for certificates issuing from the Register (for the person whose place of residence is registered at the address of the corresponding dwelling);
  • a certified by the applicant (legal representative) copy of the document confirming the authority of the person as a legal representative, with the presentation to the authorized body for certificates issuance from the Register of the original of this document;
  • the power of attorney certified in the order established by the legislation (in case of presentation of documents by the representative).

20. Why the certificate does not contain the the data of registered persons at the corresponding address?

Kharkiv Territorial Community Register was formed on the basis of information received from the Department of Registration of the Kharkov City Council from the authority of the State Register of Voters in accordance with Section II, paragraph 2 of Law No. 888-VIII. In this regard, 04/04/2016 registered citizens of Ukraine who have reached the electoral age are in Kharkiv Territorial Community Register. Therefore, information concerning the registered place of residence of citizens under the age of 18 was not contained in Kharkiv Territorial Community Register.

Please note that the details of the passport data and the date of registration of residence are not available in the State Register of Voters, therefore after 04.04.2016 uploaded data to Kharkiv Territorial Community Register, this information is not available.
