To order, you need to fill in the form displayed on the screen by entering all the necessary data in the appropriate fields, depending on who is applying - the legal representative of a child, or the applicant himself.
Filling in the "Document Type", the user chooses the appropriate type, on the basis of which the application for the procedure of data actualization is submitted:
Selecting the type "Document in which residence place data was entered", it is necessary to choose from a fixed list of possible documents types the necessary one.
The type "The document in which the data about a person's place of temporary residence is entered (reference on the application for protection in Ukraine)", is provided only for foreign citizens.
Type "Certificate of the birth of a juvenile / underage person (before reaching the age of 16)" is selected, if necessary to actualize the data of a juvenile / underage person (until the person reaches the age of 16 and receives a passport of a citizen of Ukraine). If there is additional information confirming the registration of a place of residence of a child under 16 years of age (form A, form B, which contains information about the child), it is necessary to choose - «Document confirming information about registration of the place of residence / stay of a juvenile / underage person in Kharkiv (if there is any)"
Depending on the type of a document chosen, it is necessary to enter its details, "document number" and "issuance date" are obligatory.
Depending on the purpose for which the applicant applies for data actualization (change of data, if the name was changed over the past years, or confirmation of information about himself), it is necessary to choose the appropriate block of the application for filling in.
Among the possible documents that may be the reason for the change of data, there is a birth certificate of a juvenile child, a marriage certificate, a certificate of divorce or another document confirming the change in personal data of a person (a text box with the possibility of personal data entry). For example: a reference on a change of personal data, court decision, etc. Then you need to enter the details of the selected document.
Among the possible documents confirming the accuracy of the information provided by the applicant in the case of failure to confirm it with a document, that was changed and includes information about the place of residence / stay of the person, is a certificate of birth of a juvenile child or other document confirming the authenticity of the information provided by the applicant (a text box with the possibility of entering data by yourself). Note: Filled in non-standard situations (optional). Then you need to enter the details of the selected document.
An important condition for the correct completion of the application form is to fill in the field "The essence of the application (the identity data of the person who in the last years could change -the name is indicated, or indicated the necessity to change the data about the person)." Note: the field is mandatory!
After completing the essence of the application, it is necessary to attach to the statement, scanned copies of the documents, which is the basis for the implementation of the procedure for actualization the data.
The fact of verifying the accuracy of all the information provided must be confirmed by accepting the terms and conditions indicated at the end of the form.
The procedure of updating the data on the basis of the submitted inquiries is carried out not later than the next working day, in exceptional cases, if necessary, additional requests - up to 20 working days.
